Help! Need to consolidate rows and manipulate unique data into columns at rows' end

Hey all, I'm hoping I can find some help and answers here. I'm new to the forum and have little Excel experience. I'm working with a school network and I have over 15,000 rows of data right now that I need to manipulate into a precise format for a grant application.

I'm attaching a sample spreadsheet of what the data looks like in its raw form and then an example of what it needs to look like.

The basic idea is that the data shows State test results for all students who have progressed through our school network over the last 5 years. We have three schools' worth of data. The data currently organizes each test taken by each student each year they are at our school network into an individual row on an excel spreadsheet.

part looked like it was lined up properly before I posted...It should be above the series of numbers as column headers.

Nevermind. Thanks to those that looked and took some time to think about this. After a few days of crash-coursing myself through VLOOKUP and IF statements I managed to understand the problem better, parse and organize my data in several other sheets, and create an equation which solved my problem.
