Save Multiple data by date

Hi guys

This topic is related to another thread i did Save data by date

My boss loved it and now most of us are using it. But she wanted more. Basicly the same but bit more complex. She wanted 3 boxes, in the first one she wants us to put information about the call (what people are asking about etc), the second box she wants us to put a number between 1-10, later each number will be defined and in the 3 box she wants us to put in a name of a person if we forward the call.

So basicly the same as the last thread but a bit more complex.

Can someone help me with the formula? i will attach the documnet.
