Using Parameter Query in Excel 2010 Report

I have to develop report queries on data sheet in Excel 2010. The requirements are:
User type in his/her selection in each parameter box and then submit. Data are then populated accordingly. Please see attached file for raw data, parameter selection, and query result. I tried to use MS Query but got stock at the beginning stage of importing data receiving error "too few parameters. Expected 15". Otherwise, perhaps that's the easiest method to create parameter query without writing VBA codes which I am novice at. I appreciate any help/guidance you can provide.
