how to convert xlsx file to csv file

hello everyone,
i have data in xlsx format and i want it in csv format.
how to do that.
i have tried...
1) save file as and save file as .csv
2)tried Google convertor to change the file
and
3) after rename the file and convert the file into .csv i got error while opening file that i given in attachment please go through it.
and csv file is opened with error.
i am using phpmyadmin to access database. i want to restore .csv file into table it gives me error.
can anyone help me here??

I can open the file when i change your files extension to .xlsx
How convert from .xlsx to .csv >>> click "save as", choose "save as type" as ".csv"

ya that i already do it properly before also, but format of data changes. hows that possible?
In xlsx file in that table the cells are in date time format as yyyy-mm-dd hh:mm:ss but i converted into(I mean saved to) csv file it get automatically changes to dd-mm-yyyy hh:mm:ss am/pm can you know any method to changing .xlsx to .csv file without changing the format of my data??

I am not see this question when i save file as "comma delimited".
still the same format as yyyy-mm-dd hh:mm:ss.

i am so sorry i had given first csv file in attachment, i should give you you xlsx file. And in my question i have mentioned that i want to convert a xlsx file without changing the date format into csv file.

i can't use access as my database.
i am using mysql using phpmyadmin which is by default. and phpmyadmin date time format is yyyy-mm-dd hh:mm:ss which i can't change. and phpmyadmin support .csv file only.
please help.
