Create survey in Excel with option buttons

I am trying to create a form in Excel to use as a customer service survey. The scoring is set up from 1-5 and I have created a button for each score at each question. Where my problem lies is calculating the scores and averaging them at the bottom. How can I accomplish this with option buttons?
