Linking value in external worksheet - filename varies depending on values in cells

I'm trying to extract a value from a cell in a file (J:\Offshore Wind Generation\Offshore Wind Generation File Store\GGOWL\Availability Calculations\SWP Monthly Availability\14.01\[phase 2 avail breakdown 01.2014.xls]GG Bespoke Availability'!$D$115:$F$115), and the month/year changes depending on the the value in other cells. for example, in the file name here, I have data for January 2014, and the values 01, 14 and 2014 are created elsewhere (J6 (month, I6 year and H6 respectively). When the user wants to see February 2014, the values will change to 02,14 and 2014, and the filename should then change to
J:\Offshore Wind Generation\Offshore Wind Generation File Store\GGOWL\Availability Calculations\SWP Monthly Availability\14.02\[phase 2 avail breakdown 02.2014.xls]GG Bespoke Availability'!$D$115:$F$115.
When I have the file open in another session, it links fine, but I need to create a dynamic cell reference, and I can't seem to get it done properly.

So the filename should be:
J:\Offshore Wind Generation\Offshore Wind Generation File Store\GGOWL\Availability Calculations\SWP Monthly Availability\I6.J6\[phase 2 avail breakdown J6.H6.xls]GG Bespoke Availability'!$D$115:$F$115.
