Update excel file using a form or other method

I have an excel file (see attached - access.docx). i want users to input their data using a form as per my attachment or any suitable method you could suggest and the excel file will be updated accordingly. the excel file is made up of the 11 headings as listed and there are 25 users.

Any suggestions are welcomed.
