Creating a macro to create multiple check boxes linked to different cells

I am trying to create a macro that will create a number of check boxes, which are linked different cells. I have had some success in creating multiple check boxes and having them at the destination I want the problem is that instead of linking to different cells they are all linked to the same cell. I have attached a sample workbook and any help that you could provide would be greatly appreciated.
