Vlookup picking two columns and name of the worksheet

    Vlookup picking two columns and name of the worksheet

    I know how to use Vlookup but I am unable to use it to pick 2 values, also picking the worksheet name. I am trying to match Mastersheet with other worksheets.

    Attached is the sample sheet.Column highlighted in yellow is the output I am looking for.

    I tried to test by changing the worksheetname "sheet1" to "test" but it is not working.
