Need to create a form that can accept user ID and then display list of check list items

Hi All,

I would like to create a form in excel which should accept user id & date, on accepting the user id & date - list of 5 activities with check box has to be displayed so that they can select their relevant activities and then click on submit/save, once they have put check mark on activities & clicked on submit/save, data has to be saved in excel. So that I as an admin can review who has done what activities for the day.

Note: User ID should be given by me (Admin) to the team.

Please advise with a macro code to achieve above task.

Please find attached excel. I am new to this process, not sure how to create it, but some how I have create a form with details. once any employee logs in with his ID... list of 6 check list has to be enabled so that he can select them and when he clicks on save, it should be saved in excel as shown ....

Example of user ID's 216, 219,3335,526482

Please advise..

If you have a better way to achieve this task please advise on it....
