Adding rows to list for multiple combinations

I could really use some help coming up with a quick way to alter this drug testing chart. I have attached a worksheet that has my list of data. My problem is that this needs to be laid out for using a pivot table. Currently everything seems ok except for column V "medicinal list". If you notice, there are several duplicate entries for everything except for Column R "Other drugs" and column V. Every unique "Accession Number" represents a different patient sample. Therefore duplicates are the same patient just with a different "Other drug" listed. I manually added each patient in again to have a different row for each drug with all the other same information. However, now I need to do that with column V. This column is not laid out correctly since each entry in the column doesn't necessarily correspond to each row, rather to the single patient itself. As you can see in sheet2 where I have started doing this, I need a code that could recognize the "medicinal list" for each patient and insert the patient as a duplicate just with each individual drug listed, basically showing every possible combination of "Other drug" to "medicinal list".
