Combining duplicates and grouping unique values

I am in need of help developing a code that will combine data entries. The program I use gives me an excel file that lists patients and the tests that they are ordered for multiple times. Therefore if one patient is ordered for multiple tests then that patient will be listed again for each test. I need a code that will leave only a single entry for each patient yet combine the tests that are ordered in the last column. I have attached a workbook that gives an example of a "before" and "after" in separate sheets. If possible, I would like for the code to leave the original sheet in its original form and post the output to a new sheet within the same workbook.
