Vba Code to generate a new sheet based on a template.

Hello,

I need help in creating a macro to generate blank data sheet based on a template.

So on the information Tab, I need a button for users to click to generate new tab with the blank template so they can add new data for another person.

Id also like it to be possible for the user, once clicked the generate button to be shown a dialogue box for them to enter the persons name, which will be shown on the tab when they press confirm.
