Match cells from one work sheet to index on a separate work sheet and return cost if

If Each cell A-E equals exact text compared to an index on different worksheet i want to return that cost at the end of the matched row. think code maybe be the easest way as i have tried IF/AND combinations and results in formula being too long for excel 2013 to support.
    Any suggestion?

    Les

Attach a sample workbook. Make sure there is just enough data to make it clear what is needed. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are demonstrated, mock them up manually if needed. Remember to desensitize the data.
