Issues with User Form

Hello,

I have a spreadsheet to track swimming lesson registrations. I have created a user form to make data entry more simple. I am no expert at code writing by any means, but I gave it a solid shot. Can someone please look at the code I’ve created on the attached spreadsheet, and help me correct it. Currently I cannot get the correct information linked to correct cells, or have each record begin on a new line. Also, I cannot get the button I created to run the Macro. Please someone help me!!

Next question.. Is there a way to have my userform auto-fill with data from same Userform? Example.. Say I was adding 3 children into Swimming lessons database, and they have the same last name, is there a way to have the lastname autopopulate, or will I have to type in the lastname 3 times? The reason I have txtboxes for 3 last names, is because one family some times has more than one surname.
