Problems with listbox display and search button with the userform

Hi excelforums,
I have been using a template to suit my needs which is made by merkez. The problem might be stemming from the fact that the template had only 12 columns in listbox and I have expanded them upto 32 changing a lot. In doing so I have run into a few issues regarding listbox display and search functions.
The issues are:
1. When you click on Full Listbox command button. Even though the listbox displays all the rows, the same is not correctly displayed in the textboxes img1.JPG.
The value of a column has gone missing. I believe it happens here :

2. The search button doesn’t seem to work with only the name provided. Initially only the firstname search would work. I made a few changes so that even if the full name(with spaces) is added into the search box,it should search it. But now the combobox used to select name doesn’t display. Since then I don’t get any of the search results.
3. The final problem I am facing is that, the listbox displayed in the userform doesn’t show any titles in the first row.img2.JPG.
This used to be displayed initially, but I had then added import workbook button to the userform (which I have now removed because it didn’t preserve the format of the importing workbook); afterwhich the title bar looks empty. How can I have the title bar display the titles again?

Number of studies/projects for maths is the column that has been replaced by Biology exp. And all the columns following that have replaced the previous textboxes.

Would anyone be able to help me with this?

Try decreasing the value of a in listbox.
