Vlookup & If function

Hello

I am trying to write a formula that will look at the following (example attached)

- I am writing the formula in column H ("C1") , sheet "DAR FORMAT"
- I want the formula to check the "QUERY" sheet and return the date in column M (Chg Date) if the following has been met:

1. The resident names match (Column F - "Query" and Column B - "DAR Format", the vlookup piece) AND column N on the "Query" sheet is "C1"
2. I want the date returned that coincides with the C1 CHC only, if that resident does not fit the criteria, I want the cell to remain blank

Basically, I need to lookup if the residents match, if there is a match, do any of the matches have C1 in column N? If so, provide me with the Chg Date
