Formula to copy information into another worksheet based on a particular condition

Hi there, am wondering if someone could help with either a macro or formula! I've got a spreadsheet with a table for condition assessments in each worksheet. If a condition of 4 or 5 is given in the cell for 2017 Standard, then I'd like the information in that same row, along with the worksheet name, to go into the summary worksheet. I just can't figure out a neat and tidy way to do this! I think I've managed to attach a copy of the worksheet that I've started to work with, however it's worth bearing in mind that one spreadsheet may have 20 worksheets in it, and one may only have 5 (not sure if that causes issues!).
