auto email form excel imputting specific row data for each email address + add graph

hello

I am new to macro and have completely confused myself googling "how to" and reading lots of different results. I would really appreciate if some Kind person would take pity on me and help.

I want a macro that will auto email out individual scores to individual email addresses in a data range (each row will be a new email)

And then based on Yes or no criterita in Column D auto import a graph from the same sheet.
If yes import graph
If no leave out of email

Email template
To: insert from column B
CC: insert form Column H
Subject: PPM Score

email body

Name (insert column A),

Your monthly PPM figure for ( insert Column E) is (insert from column C). You Scored has (insert Column F) your are currently ranked (insert column G)
