I would like to create a place on a bid form for a text box containing inclusions and exclusions. The two textboxes would be filled based on the Yes/No from a checkbox next to a list of items. i would like to add them with a comma and a space, and if there is a reference note add it in parentheses.

that is a good example, my understanding of code is minimal. I attached a better example of the way i would like to use the functions.
