Macro for multiply lines including MM/YY in Header

Hi, We use a large multi-sheet workbook for our risk assessments and I need to be able to put the following into the left header:

Company Name
Risk Assessments
Month / Year

Ideally, it would also need to be laid out as above.

I also need to put the following into the right footer:

Issue Date: Month / Year Next Review Date: Month / Year (this date needs to be 1 year from the Issue Date)

Can anybody help with the macro(s)? I have tried various things that I found by googling but have been unsuccessful so far.

Any help/advise would be gratefully received.

Why don't you use the macro recorder (?), then you can correct the code, for example:
