Automatic LOG for a data entered

Hello Guys,

Is there a possibility to create automatic log on one sheet (called LOG) with a data taken from another sheet (called DATA)

I have attached a simple example of what I'm looking for.

In the workbook I have 2 sheets.
One - called LOG and second one called DATA.
DATA - will be used to enter the report details and this details should be transferred to the LOG (next free line) when the button is pressed (as per attached example)
The report number will be unique , and if someones by mistake enter the same number - there should be a message informing that this number already exists.
Also if button is pressed more then one - another info saying (the event was already entered to the log, do You wish to add again?)
(button in my form does not have any macros assigned at present)

is not established case it is same as duplicate the report NO, so what's the point ?
