Trying to create a macro with pop up box for selection from table

Hello,

As I am new in creating macros I need a little help.

I have a file with open orders in my system, and i need to filter them by specified family and client. I have done almost everything I need but I am stuck on one option.
At the begining of macro I need to have a pop up box, where user can chose the client and family that he want to see the open orders:
It is like that:
In column B I hava the client and in column D i have families. And as neewbie in VBA i have no idea what should I do, I will be grateful for any help and tips

Help us to help you - upload a de-sensitised copy of your workbook - otherwise it becomes a drawn out guessing game.

Please attach a sample workbook (not a picture or pasted copy). Make sure there is just enough data to demonstrate your need. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are shown, mock them up manually if necessary.

Remember to desensitize the data.

Click on GO ADVANCED and then scroll down to Manage Attachments to open the upload window.

I got a little to fast, and did not think of sharing the file. I have attached it.
In file sample it the before, and in sampe 2 is after. I have done the final result but just chossing the families and clients in the code, but I need to have the option to choose them by pop up box, as not only me will be using this macro.

Give the attached a trial - green button top right-hand side will re-launch the form when/after you have dismissed it.
Some of the column headers in the second sheet did not coincide with those on the data sheet so no info passed to the query sheet.
Example;- Column 'A' - where does the time come from ?
