    Add Blank Tables Upon Failure

I am trying to add a blank table below the default table upon failure anywhere inside that default table. I have the VBA set up with appropriate triggers, I just don't know where to start. I have filled out my example spreadsheet with what I would like for it to look like. Notice for Equipment 1, upon failure in the first table, it adds a new blank one, replacing the top line with a "Notes" section. For Equipment 2, it fails twice, so there are a total of three tables. I think we can just copy over a blank table every time, so I put on in the second tab to use for that. I'm sure it's an easy feature, I just don't know how to do it. I appreciate any help you can provide in working this out!
