Problem in embedded chart in userform

In the attached file, a Column Clustered Chart according to the ALLMonthPlot array with 12 elements as x-axis and MeanOfPureRate as y-axis should be displayed, and name of selected in ComboBox should be written as Title above chart, but the names of more columns are next to the picture and the graph is wrong. What is the problem?

How to add the label of each bar that represents the number in MeanOfPureRate array, above it?

I think you have attached the wrong file.
