make a mailing list that sends only one piece of mail to each couple listed

I need to make a mailing list that sends only one piece of mail to each couple listed. I've already isolated those pairs of people that have the same address and last names. My list looks like this, one name per cell:
    Sam Smith
    Karen Smith
    Alfonso Wilson
    Judy Wilson
    Alice White
    Doug White

etc., etc...for thousands of names. Each pair has the same address a few cells over to the right.

What I want is to be able to do is merge a letter that's addressed to "The Smith Family" and salutation-ed "Dear Sam and Karen". I can do this if the list looks like this (I'll end up deleting the lines without first names):

    Sam Smith Karen
    Smith
    Alfonso Wilson Judy
    Wilson
    Alice White Doug
    White
I just need to move the one name up one row and to the right a few cells. Here's a macro I used last year to do this...it worked then, but I must have changed something...what gives? The macro only starts on the 10th row, names are printed all over the sheet....oy.
