Userform buttons not working on dynamic form

I am working on a dynamically produced userform but the Cancel and OK buttons I created aren't working. They appear to be set up the same as other command buttons I have that are working perfectly. The only difference is the dynamic creation. Looking at the Locals info it all appears correct:

What exactly isn't working? From the code you posted it looks as though the buttons are created and placed on the form. Are you having a problem with the buttons appearance or that the buttons aren't executing any code when clicked?
