Script Out Of Range Error at Logon

Hello,

I need help in setting up the admin user bdb on this workbook.
I don't know how to correct the error I get when I try to logon
as this user. I've attached the file.

I'm having problems with the other logons.
Every user except the administrators is giving me an error.
Here's what happens:
1. Log on w/username at UN & PW prompt box
2. access granted after logon attempt
3. Error msg box pops up stating: Run-time eror '9'
subscript out of range.
4. Select Debug
5. The VBA code has the following highligted.
