Error shown when opening Excel Form with Userforms

Good Afternoon All,
I am hoping you can assist me with this?

I have a file that i have had help with from these forums, File is attached.

My problem is, i can open this file on my home PC, My work PC and my colleagues work PC, however we have a further 5 PC's in the office at work and this file WILL NOT work on them. The PC's are exactley the same, with the same version of Excel and the macro security set at the same.

The file opens, You try to log in and it comes up with the VBA Screen rather than logging in as it does on my PC.

Can anyone assist with this? It is driving me nuts!

PS: The Username is TEST and Password is TEST on this file
