feeding forms into worksheets

Can someone please help with this. I am trying to design marcros / formulas for the attached worksheet I have designed, but as I have never used marcros finding it very difficult. I have inputted some but I am now completly stuck.

In other words I am trying to make all the forms work and feed into the worksheets correctly.

Any help would be greatly appriciated.
