Chart Data to fill in spreadsheets

Hello all,

Attached is a sample workbook which comprises of a "Form" & a chart of data.

I cannot figure out how to copy the form to a new sheet and then populate the matching fields from the chart.

Each row on the chart needs to be a new filled in Form.

I would prefer if the Master form stayed blank but if not then no problem.

Each sheet can be renamed using the last name and all the sheets can be in the same workbook.
