Employee - Self Appraisal

Hi Gurus,

Can anyone please help me with the below requirement

This relates to employee appraisal. I've all the parameters ready. I need to have a submit button at the end of the work book. Once the employee is completed appraisal and when clicks on submit, it should trigger pop up asking for recommendation / suggestion (not mandatory to be filled in). Once the employee click ok, an e-mail has to be triggered to his/her manager for review. The manager review the submitted self appraisal and make changes, if required. Once the discussion is over, the manager clicks on submit button with final rating. This should trigger an e-mail to the employee with the final rating and recommendations from the manager
