add certain text in a cell between the words

hi,
I'm new to excel.
I want a macro to add certain text in a cell between the words...
In the attached example..
The cell in blue color should display the text which is there in the B1,B2 and if the user selects the Hindu radio button then hindu should be displayed and if the users selects the graduate option the that should be added and finally the comments should be appended in the blue cell when user clicks submit button....
The text in the blue color should look like this....
"Bobby is 18 years old. He is a Muslim. He lives in Bombay. He has done graduation from xyz college."

For you other forms, you would also have to do cell links and incorporate that into the formula. If you unprotect the sheet, I could have a look again.

I have not protected the sheet..
when i click on submit button the data should appear in blue cell and i should have one more button called append beside to submit button and when i click the append button i should be able to append the data from the cursor position.. in the blue cell...
