Inserting Header and Page number in Word Document

here is my Excel macro. It copies the data from the current sheet to a Word Document

I need to add a Header(Logo picture on the left of ONLY page 1); a footer(Page number starting at 1 from page 5) and also Table of contents on page 4 (capturing text starting page 5)
