User Form VBA coding to enter, edit and add new records

Hello everyone, I am new to this forum and excel VBA. I am working on a project for someone and have been handed this awful task of putting together a User Form to enter, edit and search for records. I have attached a file showing what we need it to look like when it is being used. I have used excel previously for basic data entry, but when it comes to something like this, I have absolutely no idea where to start. I do not have access database on the computer available to me or to the person that needs this.
Would anyone be able to help me with this? I'm very desperate! Thank you for reading and apologies if this post has ended up in the wrong place.

Hi there, I have attached the table and a line of sample data. The unique reference no. in the first column is provided by a different organisation so I have to be able to input and not have it as an automatic allocated number. Also, not necessarily all of the boxes for the names may be completed, there are occasions where they may remain blank but I may need to update them at a later date.

I forgot to mention, I only have excel 2003! *Bites Nails*
