Replacing Range of Values with a Matched Range of Values

I receive a database weekly with ~ 300 +/- records. I typically filter it down to 100 - 125 records. I publish a separate, more concise table (than the original)with this data, and would like to replace longer descriptors of causes for delays in this "new" table with abbreviated versions without having to do it manually. I've looked into VLOOKUP for replacing values, but I can't figure out how to get it to replace one value (out of a range of values in col A) with a matching, abbreviated descriptor (out of a matching range of values from col B). I've attached a simplified table of the values to replace (col C) and the values to replace them with (col D). Would muchly appreciate any guidance. My first post, so hope the Atch goes thru. Where do you want to do the replacements? In the All records sheet? Or somewhere else?
