VB Macro to insert value in cell based on multiple criteria in a row

I am requesting assistance for A VB macro that will insert a value in cells based on multiple criteria in the same row. Please view Excel file on attachment and read text Steps 1 and 2. Instructions are very easy to understand with visuals and cell colors.
