how to create word document based on data in excel

Hi all,
could you please share with me how to prepare vba macro which will create new word document as copy of existing word template , then will update 3 fields based on data in excel and will save this word to specific location with specific name (both name and location should be parametric based on data in excel).

This macro should run from the excel with data.
