Command Button to Call User Form - Run Time Error 1004

This has been working in the past versions. All my other Command Buttons to call User Forms are working correctly.
Command Button calls this Sub:

The User Form is named: FlatSvce

Error:
"Run-time error '1004':
Application-defined or object-defined error"

Would the error be in the code for FlatSvce?
