    Create report based on user options

    Hi,
    I need a worksheet to create reports based on what the user options selects.
    I have a sheet with all my data and a sheet where the user will select what he needs/wants in his report, when the user clicks submit a new sheet should be open with the options selected.
    The data will be updated every month so the population and other fields could change and options, the fields will be the same every month.

    In the options there are some that must be selected always, others are optional and other are optional with multiple values.

    I attach a example file.
