Creating Database from Input Form

Hi everyone, just joined this forum. I'm trying to get some help creating a table, where the data is inputed through a form on the first sheet. Every new form that's filled out then creates a new row in the upload sheet. Does anyone have a solution for this?

I'm trying to keep organize a hotels group and catering accounts receivable and credit functions.
