I need one line of information to compile into a master spreadsheet

I am not that great with excel and am trying to figure out how to do something. Basically I need either a pop up or an excel sheet to have some important information for people at work to log in samples. So the required information is

Initials, Notebook #, Page #, Date & Time sample submitted, Concentration, Rush, Length, Diameter, Particles & additional comments

So basically I got as far as getting the date and time submitted to automatically post when they entered in their initials. I also had check boxes for the rush, length, diameter, and particles part (this is what they're asking the analytical team to do for work).

I was trying to create a macro that once you click a button all this information would automatically compile into another sheet and reset the information on the first sheet for another person to enter another submission. Then when the next person submits something it fills in all the information one line below the first persons submission keeping a running master sheet of every submission that's ever been done. My problem is either I can get it to move down to the next line or I can't get it to transfer any of my numerical data in the original entry to the master sheet.

Any help would be greatly appreciated. I attached what I currently have

    Thanks

Ideally I guess a pop up window that had all this information for logging a sample in would be better as well as space to enter up to 8 samples at a time. One sample at a time would be fine but this is way above my excel talent. Thanks again for anyways that can help.
