Run-time error 9 Subscript out of range

I am creating a user form in excel VBA. I have just added some check boxes to this and I am now getting the following message

"Run-time error 9 Subscript out of range"

Why is this?

Hi Katie,

aside from taking wild guesses to help (a pretty non productive method of problem resolution) there isn't much we can do without a sample workbook that is causing the error.

If you post one then you get approx 1000% increase in chances of help

Please see attached. For obvious reasons I have needed to amend lots of data so I am hoping I haven't removed anything significant in the process.

I do apologise, I am not sure what I have done but there is no longer an error for this.
