User form on initialize to recall last entry from Excel Sheet

Hi, I have a user form that posts information to a data sheet, this is done daily. I want the added function of the user form, when initialized by the user, to retrieve (from the data sheet) and show certain values (in rows 5,8,11,14,17&20) that were entered in the day before. I am a novice at this stage and don't know where to start. A copy of the Excel file is attached, please help!
