Input Box HTML Body

The below code is what I use in one of my macros to reply to a message within Outlook. How can I set it up so that a box pops up and asks me what I would like the value to be where "12345" currently is, and another box to ask me what I would like placed below "Response:"?

Please see attached for example of the code. For some reason I am being restricted by the site or my work from including code in my comment.

Never mind, I figured it out. I was missing a small step.
