Pivot Table Survey Data Multiple Questions, 5 Responses

We just started a survey form for our system and I am have been working for days trying to figure it out.

We have 5 Questions with 5 (6 if no reply) different possible answers for each group. I would like to group the count for the answers at the top but when I try that it just does a drop down or when I try to merge it doesn't work.

I saw that you can put it into a Flat Table format but I don't know how to do that.
