Need to count different values in multiple columns

I am trying to count billing codes that appear next to each other in multiple columns. Some of the codes need to be counted as a group, or package, while others may not. Think in terms of a maintenance guy recording that he performed tasks 1, 2, and 3 on a work order. If he did tasks 1 and 2, then they fall under a package deal, while task 3 was a la carte. These tasks would be coded in their own column, in no particular order, side by side. In this example, I'd have a count of 2 billable items rather than 3. A more detailed example is attached.
