    Creating a Word Document with a submission form that is saved as a PDF

    Is is possible to create a Word Document that would include a web submission form that when saved as a pdf would work when accessed from the web?

    There is a sample attached. Created in Office Word 2007

    You cannot save a Word document as a fillable PDF form at all.
