What kind of form is this (eg userform in a document, or a document using formfields, content controls or ActiveX controls)?

Building a string from the leading characters of other strings is a trivial exercise, using the Left function [eg: username = "vie" & LCase(Left(2ndname,2) & left(name,1))]
