Help with populating a performance grid with excel data

Hi, I am looking to do a performance grid, similar to a 9 box, with excel data. I tried to post a link to the original thread where I saw a solution similar to what i wanted to do but received an error . I tried to modify it but it's not working properly. I would want to use it as a template where only the employee name and function are modified, and the rest is "locked"

There are a few modifications I would want to make, and have outlined on the attached.
- do not need the slicer for Levels, only need to slice by function
- want to change the title of "talent talk" to say "Rating"
